Patient Portal

We encourage you to sign up for our Practice Portal.  In the face of information theft, we need a secure and practical means of corresponding and protecting sensitive data.  Our online portal allows you to utilize several services anytime, anywhere.  These services include:

 

  • Submit New Patient and Return Patient paperwork online

  • Appointment and Prescription Refill Requests

  • Update Demographic Information

  • Financial Statement Viewing

  • Access to Medical Health Information, including a Continuity of Care Document

  • Ask a Patient Care Coordinator or Provider questions

  • Submit a Referral Request

  • Secure Messaging with the Practice Staff

  • Send Medical Records through a secure server

 

Safeguarding your information is a priority to us.  Secure messaging is a solution to ensure the protection of personal and confidential information and to facilitate government compliance.  To ensure the security of your personal information, we use industry standard encryption to prevent unauthorized access to data.

 

The portal will perform the best when used on a Microsoft Internet Explorer (v6.0 or above) browser.

Instructions

 

Please complete your registration by:

 

  1. Visiting http://medicalexerciseassoc.portalforpatients.com (do not type "www") or clicking the image above.

  2. On the top left hand side of the screen please click register.

  3. Fill out your name, create a log in and password, and enter two security questions.

  4. On the next page, please select the option that states "I am already a patient or I already have an appointment."

  5. You will then be asked to verify your identity. Please reference the email sent by our office for this number, or call and request your personalized pin number.

  6. Once you have verified your identity, please select the tab "Online Patient Forms".

  7. New Patients: Please complete the New Patient & Family History, New Patient Injury History, New Patient Review of Systems, New Patient Medical History, and the New Patient Surgical History. New Auto Patients: Please complete the Auto Patient’s Form in addition to the other new patient forms. 

 

        Return Patients: Please complete the Return Patient Form.
